- Applicants must be businesses located in the City of Mesa. Verify your location here: (EXPLORE MESA).Tip: You can then copy the address returned into the application.
- Business must have suffered negative financial impacts or business interruptions due to required business closures or limited service due to COVID-19.
- Provide a signed W-9.
